Format for Site Visit Report

This document is to serve as a guideline for an ASHA member doing an evaluation of a project through a site visit. The project may be currently being considered for funding, or may have been approved for funding and the site visit is an interim evaluation. The questions mentioned in the guideline are meant to be encompassing, and it is natural that the member may not be able to collect all the information mentioned in the form.

The report can be divided into 4 sections:

1. Preliminaries
2. Statistics
3. Observations
4. Opinions

Preliminaries

1. Name of organization and location:
2. Date Visited:
3. Describe details of arrangement of visit:
   • Was it a surprise visit,
   • What was the accessibility of the location,
   • What was the attitude of the staff for the arrangement of the visit
4. Additional relevant information on the objectives of the organization (Attach brochures, etc.)
5. Additional relevant information on the operation of the organization (Attach brochures, etc.)

Statistics

1. Statistics on number of children, male-female ratio
2. Statistics on staff - number of paid, unpaid, full-time, part-time
3. Expenses, sources of income (vouchers/receipts to be attached, if possible)
   Any changes in this from the original application? If so, why?
4. How were the funds sent so far used? How will they be used?
5. Any signs of work in progress, or work in planning phase?
6. Sources of information of 1-5 above

Observations

1. General comments on discipline/class behavior of children
2. General comments on discipline/class behavior of staff
3. Comments on study environment, recreational facilities and site for occupational training for the children (Photographs are highly desirable)
4. Details of interviews with children, parents, staff
5. Impressions of the neighbors on the organization
6. Miscellaneous impressions

Opinions

1. In the site visitor's opinion, why should ASHA fund this project?
2. What reasons, if any, does the site visitor see for ASHA not to fund this project?
3. What improvements, if any, would the site visitor suggest to the project?
